This position provides case management services to homeless veterans, veterans at risk of homelessness, and their families through the Operation Military Assistance Program (OMAP), coordinating VA benefits, other public benefits, temporary financial assistance, and supportive services needed to help clients obtain and maintain housing stability.
Responsibilities:
• Receive referrals of potential clients, conduct Initial Client Visits, and determine and document SSVF program eligibility based on veteran status, income, and housing status • Assess client needs, help clients set goals, develop individualized housing stability plans, and monitor progress • Recertify participant eligibility on a periodic basis as required by SSVF regulations • Work with regional, state, and local VA programs — hospitals, outpatient clinics, VET centers, and Veterans Benefit Units — to coordinate VA benefits • Work with state and local nonprofits and community-based organizations to coordinate other public benefits and services • Coordinate supportive services and temporary financial assistance, including rental assistance, utility fees and deposits, moving costs, emergency supplies, transportation, and child care • Act as a client advocate to resolve emergency problems and assist clients in crisis situations • Maintain case records to professional standards and enter client information into the Homeless Management Information System as required by the VA
